Description of Program and Relief
Care4Kids Child Care Assistance
Wisconsin’s Care4Kids child care assistance program delivers coordinated, comprehensive health care including physical, behavioral and oral health care that is tailored to each child's individualized needs.

Eligibility Requirements/Restrictions
The Care4Kids initiative is offered to children who are placed in out-of-home care in Kenosha, Ozaukee, Milwaukee, Racine, Washington and Waukesha counties. An enrollment specialist provides information to the child's parents regarding the voluntary nature of the program, the parent's right to disenroll at any time, and the benefits provided through Care4Kids. If the parent agrees to have the child enrolled in the program, then Children's Community Health Plan will provide the parent and the foster parent with more information.
